A dependent care FSA receipt for every family, ready when they ask

What a claim commonly asks

The details a plan usually wants the provider to show

Plans differ, so check the plan's claim form. These are the items claims commonly ask for, and where each one sits on a Tuitionslip statement.

  • Provider name, address and phone
  • The provider's tax ID, printed in full
  • Dates of service: the period, with every payment by date
  • The amount paid: monthly subtotals and a total
  • Who the care was for: the family and the children you add
  • A signature line and date for the provider

FAQ

Questions about dependent care FSA receipts

What does a dependent care FSA claim usually need from the provider?

Plans differ, so check your plan's claim form. Commonly asked: the provider's name and address, often a tax ID, the dates of service, the amount paid and who the care was for. Some plans also ask for a provider signature.

Does a Tuitionslip statement have those fields?

It carries the provider name, address, phone and tax ID, the family and children you enter, the period, every payment by date with monthly subtotals, the total, and a signature and date line. Any family can get the same figures titled as a childcare payment receipt (dependent care).

Will my plan accept it?

Tuitionslip cannot say what a particular plan accepts. It is a payment record, not a plan form. Check the plan's claim instructions or ask the plan administrator.

Is this tax advice?

No. Tuitionslip makes payment statements. Questions about what a family can claim go to their tax preparer.

Is my file uploaded?

No. Everything is read and made inside your browser tab and nothing about you or the families is stored.
