What the statement contains
A daycare year-end statement parents can keep
Plain black on white, so it prints cleanly on a home printer. The sample is made up: a fictional provider and family.
- Your business name, address, phone and tax ID
- The family name and, if you add them, the children
- The period: January 1 to December 31 of the year
- Every payment by date, grouped by month, with monthly subtotals
- Total paid for the year and the number of payments

FAQ
Questions about childcare tax statements
What is a daycare year-end statement?
A one-page record of what a family paid you during the year: who you are, who paid, every payment and the total. Parents commonly keep it with their dependent care records when they ask a provider for a daycare tax statement. Tuitionslip makes the statement; it does not file anything for anyone.
Is this a tax form?
No. It is a payment statement, a record of what was paid. It is not tax advice and it is not prepared for any agency. What a family does with it is up to them and their own tax preparer.
What files can I use?
A Venmo statement CSV, a PayPal activity CSV, a bank or Zelle export CSV, or a simple sheet with a date column, an amount column and a payer column. Tuitionslip looks for the header row itself and keeps completed incoming payments only. If your bank labels things differently you can paste the rows or fix the names on screen.
Is my file uploaded?
No. The file is read inside this browser tab and the PDFs are made there too. The page makes no request that carries your file, your tax ID or any family name, and the Tuitionslip server stores nothing about you or the families.
Why is one family free?
So you can see a real statement for one of your families before you pay anything. The season pass is for providers who have a whole roster to finish in January.
Can I add my own signature?
Each statement has a signature line and a date line. Print it and sign, or sign the PDF in your PDF viewer.
What if a payment is missing or wrong?
Open the family's payments, untick a row that is not childcare, or fix the file and read it again. For a payment that never went through Venmo or your bank, use the one-family form and type it in.
Can I get a refund?
Full refund within 14 days of purchase: email support with your receipt.
