For home daycare providers

Give every parent the daycare year-end statement they asked for, in about 2 minutes each

In January parents ask for your provider name, address, tax ID and what they paid. Add the payments, or read your Venmo, Zelle, PayPal or bank file, and download a clean statement for each family.

One family free. All families $19 per season.

  • One family free, no sign-up
  • Your file is read in your browser, never uploaded
  • $19 makes every family, with a ZIP

What is on it

The fields parents are typically asked for

Every statement carries the details a parent usually needs when they gather their childcare records. The sample above is made up: a fictional provider and family.

Provider name, address and tax ID

Your business name, address, phone and EIN or SSN at the top, printed in full on the PDF.

Dates and amounts paid

The period, every payment by date under its month, monthly subtotals and one total for the year.

The child and the family

The family name and, if you add them, the children, so the statement names who the care was for.

How it works

From a payments file to a statement for every family

  1. Add your details

Business name, address, phone and tax ID once.

  2. Add the payments

Type in one family, or choose the Venmo, Zelle, PayPal or bank file and the tax year.

  3. Download the PDFs

One family is free. The season pass makes a PDF for every family and a ZIP of all of them.

Provider information

The provider details parents need for Form 2441 and Form W-10

These are the IRS pages parents are usually pointing at when they send you the January request.

The IRS instructions for Form 2441 (Child and Dependent Care Expenses) say: “You can use Form W-10, Dependent Care Provider's Identification and Certification, or any other source listed in its instructions to get the information from the care provider.” Read the full text in the Instructions for Form 2441 at irs.gov.

Form W-10 is the IRS form a parent can hand a provider to fill in with their name, address and taxpayer identification number. See About Form W-10 at irs.gov for the form and its instructions.

A Tuitionslip statement puts your provider details next to the payments the family made, so you answer the request with one clean page. It does not replace Form W-10, and what a family should claim or report is a question for their tax preparer.

FAQ

Questions about daycare year-end statements

Is a Tuitionslip statement Form W-10?

No. Form W-10 is an IRS form a parent can ask a provider to fill in. A Tuitionslip statement is a separate payment record with your details, the family, every payment and the total. Some parents want both; the statement does not replace the form.

Does the IRS require this statement?

Tuitionslip is not an IRS product and makes no claim that any agency or plan accepts a statement. It is a clean record of what a family paid you. For what a family needs to file, ask their tax preparer.

Which payments does it include?

Completed incoming payments from the Venmo, PayPal, Zelle or bank file you choose, or the payments you type in. You see every family and every row before anything is made, and you can untick a payment that is not childcare.

Is my tax ID uploaded?

No. Your details, the families and the payments file are used inside your browser tab to make the PDFs. They are never uploaded or stored.

How many families can I make for free?

One. The season pass is $19 for the season (one tax year) and makes a PDF for every family in your payments file, plus a ZIP of all of them.
